Ever wish medication management for older adults came with a simpler roadmap? An initiative intended to help older adults and those who care for them navigate the complexities of managing medications has been established in collaboration with The John A. Hartford Foundation, Peter Lamy Center on Drug Therapy and Aging, and the American Society of Consultant Pharmacists (ASCP).

This initiative, called Leveraging Pharmacists as Age-Friendly 4Ms Champions, will form a national advisory council, create a medication optimization learning collaborative across eight regions, and train 2,500 pharmacists and 950 long-term care pharmacies!

Pharmacists are one of the most accessible health care practitioners, and as the medication experts in the healthcare system, they are perfectly positioned to help older adults gain the most benefit from age-friendly care.

Illuminate (powered by PharMerica) is offering a great webinar opportunity titled “Age-Friendly Care in Post-Acute and Long-Term Care Settings: Leveraging Pharmacists as Part of the Interdisciplinary Team” on Tuesday, May 27, 2025, at 1:00 pm ET. What a great opportunity for providers!

This program is funded by The John A. Hartford Foundation and will focus on fostering pharmacy champions in the 4Ms through medication optimization in nursing homes, assisted living communities, geriatrics primary care practices, and other community settings.

I highly recommend that your group register for this enlightening webinar and invite your physicians, physician extenders, and pharmacy representatives. Please see the registration link below. We can all learn together, as one of the goals of Age-Friendly Care is to have an interdisciplinary approach!

Stay well and stay informed!
