The NHSN surveillance definition of “up to date” is updated quarterly (if needed) to incorporate FDA and CDC vaccination guidance changes. With ACIP’s February 2024 recommendation that everyone aged 65 years and older receive a second dose of the 2023-2024 updated COVID-19 vaccine, the NHSN surveillance definition of up to date was update on April 1, 2024, which is the beginning of the second quarter of 2024. The dashboard now reflects the new definition of up to date, as of the reporting week beginning April 1, 2024.

Per CDC data, as of March 31, 2024, 42% of nursing home residents were “up to date” with COVID-19 vaccines, however, the weeks of April 7, April 14 and April 21 show a significant drop in residents identified as “up to date”, 30-31%. Not to rock the proverbial boat, add to these statistics, nursing home staff who are considered up to date average 10%! What does all of this mean? When does the COVID alarm stop sounding? Many residents in our centers are affected by RSV and Influenza every year. We calmly offer flu and RSV immunizations and move on, hopefully that will be the same with COVID down the road.

Now, bring on measles! Measles is an airborne infection and if you suspect a case in your building, you must use an airborne infection isolation room. Wait, don’t have one? Most facilities don’t! As we promote the use of sponsoring nurses from other parts of the world to fill the gap in staffing, measles could become more troubling. Yikes, what are providers to do? Lots of moving pieces, so let’s end it here with a couple ideas:

  • Is your infection preventionist on top of the game?
  • Is your current infection prevention program up to date with the newest and greatest information?
  • Are you verifying vaccination data upon admission?
  • Are you monitoring residents and staff for signs and symptoms of active infection?

If you aren’t sure, make sure. Gather your team, ramp up the game, and continue to encourage vaccines! Stay well and stay informed!
